According to the Law of Ukraine “On Administrative Services” of 06.09.2012 №5209-VI, hereinafter the Law:
Administrative service – the result of the exercise of power by the subject of administrative services at the request of a natural or legal person, aimed at acquiring, changing or terminating rights.
The requirements of the Law apply to the provision by the subjects of administrative services of extracts and extracts from registers, certificates, certificates, copies, duplicates of documents.
The list of administrative services is determined by law (Article 5). – issuance of a certificate of divorce;
– issuance of a certificate of registration of name change;
– issuance of a certificate in connection with the change, addition, correction and renewal of civil status records;
– re-issuance of certificates of state registration of civil status.
Free administrative services include:
– issuance of a death certificate;
– issuance of a birth certificate.
The department has developed information cards for each administrative service.
According to Part 3 of Article 20 of the Law of Ukraine “On State Registration of Civil Status Acts” departments of the DRACS provide, in addition to administrative, paid services, the list of which was approved by the Cabinet of Ministers of Ukraine 681168 of 22.12.2010.

1. Department of state registration of acts of civil status in the city of Korosten’s registration service of the Korosten City Department of Justice in Zhytomyr region (Next-department) is a structural unit of the registration service of the Korostensky City Office of Justice in Zhytomyr region (hereinafter referred to as the Registration Service of Justice), which implements the powers of the state Registration Service of Ukraine (hereinafter – Ukrderzhrejestr) in the field of state registration of civil status.
The department reports directly to the head of the registration service of the Department of Justice.
The department is created, reorganized and liquidated in accordance with the order of the Ministry of Justice of Ukraine.
2. The Department is guided by the Constitution of Ukraine, laws of Ukraine, decrees of the President of Ukraine and resolutions of the Verkhovna Rada of Ukraine adopted in accordance with the Constitution of Ukraine and laws of Ukraine, acts of the Cabinet of Ministers of Ukraine, orders of the Ministry of Justice of Ukraine, other legislative acts. 3. The main tasks of the department are:
3.1. Ensuring full, timely and correct state registration of civil status acts in accordance with the requirements of the legislation.
3.2. Ensuring the realization of personal and property rights by citizens within the defined competence.
3.3. Preservation of the archival fund of the department of state registration of civil status acts.
3.4. Maintenance of the State Register of Civil Status Acts in compliance with the requirements of the current legislation.
3.5. Issuance of certificates of state registration of civil status.
4. The department in accordance with the tasks assigned to it:
4.1. Conducts state registration of birth of an individual and his origin, death, marriage, divorce, change of name (surname, first name, patronymic) and for each fact of state registration of civil status issues the relevant certificates of state registration of civil status.
4.2. Enters into the State Register of Civil Status Acts information on the birth of an individual and his origin, adoption, deprivation and renewal of parental rights, marriage, divorce, change of name, death, as well as changes, renewal and cancellation of civil status records in accordance with The procedure for maintaining the State Register of Civil Status Acts, approved by the resolution of the Cabinet of Ministers of Ukraine of 22.08.2007 N 1064 (as amended).
4.3. Prepares materials and decides on changes in the name (surname, first name, patronymic) of individuals – citizens of Ukraine.
4.4. Prepares materials and decides on amendments to civil status records and renewal of lost records.
4.5. Prepares and submits to the Department of State Registration of Civil Status Acts of the Main Service of Justice of the Main Department of Justice of the Ministry of Justice of Ukraine in the Autonomous Republic of Crimea, main departments of justice in oblasts, cities of Kyiv and Sevastopol. and re-drawn up civil status records.
4.6. Draws an opinion on the annulment of the marriage record in the cases provided for in Article 39 of the Family Code of Ukraine, and annuls the civil status records in accordance with the law.
4.7. Decides on the withdrawal and return of the revoked certificate to the relevant body of state registration of civil status. In case of non-return of the revoked certificate, it provides information on its revocation for publication on the official website of the Ukrgosregister.
4.8. Upon acceptance of the application for state registration of marriage at the request of the bride and groom issues a referral for a medical examination.
4.9. Ensures proper storage and execution of books of state registration of civil status acts, metric books and other documentation related to the state registration of these acts.
4.10. Maintains in the prescribed manner the books of state registration of civil status.
4.11. Ensures proper accounting and storage, as well as the use of forms of certificates of state registration of civil status in compliance with the law.
4.12. Re-issues certificates of state registration of civil status acts, extracts from the State Register of Civil Status Acts of citizens in the manner prescribed by current legislation of Ukraine.
Registration of births and places of residence of newborns.
Korosten City Department of State Registration of Civil Status Acts of the Main Territorial Department of Justice in Zhytomyr Oblast, according to the order of the Ministry of Justice of Ukraine “Some issues of organization of work of departments of state registration of civil status acts related to state registration of childbirth” / 5, (as amended) informs that the department cooperates with the maternity ward to provide services for receiving documents required for state registration of birth and issuance of birth certificate directly in the maternity hospital and, in addition, receiving documents for registration of residence of the newborn children.
The state registration of birth is carried out on the basis of a medical birth certificate issued by the medical institution where the birth was performed.
To submit an application for state registration of birth and obtain a birth certificate directly in the maternity hospital, in addition to the medical birth certificate, you must provide the following documents:
– if the child’s parents are in a registered marriage and have a common surname: parents’ passports and marriage certificate. The presence of a father is not required;
– if the child’s parents are in a registered marriage and have different surnames: parents’ passports, marriage certificate and a joint application for the child’s surname (the application is made when submitting documents). The presence of both parents is mandatory both when submitting the application and when receiving the birth certificate;
– if the mother is not in a registered marriage, but there is a father who wants to recognize paternity: parents’ passports and a joint application for recognition of paternity (application is made when submitting documents). The presence of both parents is required both when applying and when receiving a birth certificate;
– if the child’s mother is not in a registered marriage and there is no application of a person who recognizes his paternity: mother’s passport and application for recording information about the father in accordance with part one of Article 135 of the Family Code (application is submitted when submitting documents).
In addition, for the state registration of birth, citizens can personally apply to the department of state registration of civil status at the place of residence or location of the maternity hospital.
To obtain a certificate of registration of residence of a newborn child, in addition to the above documents, you must submit an application for registration of residence of a minor child, a receipt of payment of administrative fees. If the parents live at the same address, the presence of one of the parents is sufficient when submitting the application. If the parents do not live at the same address, both parents must be present when applying.

In accordance with the order of the Cabinet of Ministers of Ukraine dated July 13, 2016 № 502-r “On the implementation of a pilot project on state registration” (as amended), in the city of Kamianets-Podilskyi introduced state registration of marriage in a short time. Persons wishing to register a marriage in a short time must enter into an agreement on the provision of services for the organization of state registration of marriage – with Kamyanets-Podilsky State Historical Museum-Reserve, submitting the following documents:
1. Passport of a citizen of Ukraine, passport document of a foreigner or a document certifying a stateless person (foreigner, stateless person, attaches a document confirming the legality of his (her) stay in Ukraine).
2. If the persons wishing to register the marriage, or one of them was previously married, – documents confirming the termination of the previous marriage or annulment of the marriage (certificate of divorce, court decision on divorce, annulment of marriage, which has entered into force, the death certificate of one of the spouses, the conclusion of the Department of State Registration of Civil Status on the annulment of the marriage certificate, which is invalid, etc.).
The originals of the documents confirming the termination of the previous marriage are not presented, if the relevant civil status records, which are notified in the contract, are drawn up by the bodies of state registration of civil status acts of Ukraine.
3. Documents drawn up in a foreign language shall be submitted together with their Ukrainian translations, certified in accordance with the established procedure.
4. Documents on payment of state duty and payment for paid services provided by the Department of State Registration of Civil Status Acts.
The state registration of marriage is carried out within the period specified by the applicants, including on the day of submission of the relevant application.
